Debunking the Myth: Are Almonds Really "Poison" for Certain Health Conditions?

Almonds are called the powerhouse of nutrients. It contains calories, protein, carbs, dietary fiber as well as vitamin E, vitamin B2, magnesium, phosphorus and potassium

Many people have a handful of almonds as a healthy start to their day. Often, people will soak their almonds overnight or eat them raw. Almonds can be included in many foods due to their high content of vitamins E, magnesium, fiber and healthy fats. This can include smoothies, breakfast bowls and desserts.

However, there have been claims surfacing online suggesting that almonds are toxic. Almonds are not toxic to the majority of people and can easily be incorporated into a healthy, balanced diet. However, some people may experience problems if they suffer from certain allergies, digestive issues or kidney issues.

India's ICMR-National Institute of Nutrition (ICMR-NIN), in its 2024 Dietary Guidelines for Indians, recommends including a variety of nuts and oilseeds in the daily diet. Its illustrative balanced-diet plate includes 35 g of nuts and seeds as a combined food-group quantity, not 35 g of almonds alone. However, nutritional benefits do not mean almonds are suitable for everyone in unlimited quantities, particularly people with specific allergies or medical conditions.1

We must first identify whether we are eating sweet or bitter almonds.

Are Bitter Almonds Toxic? Understanding Cyanide Poisoning

Most of the almonds we buy from supermarkets are sweet almonds. Sometimes, there can be confusion between sweet and bitter almonds.

Bitter almonds contain a natural substance called amygdalin. This can be broken down in our bodies to produce hydrogen cyanide, a highly toxic substance that prevents us from using oxygen.

In 2024, a patient suffered with severe cyanide poisoning after eating approximately 40 bitter almonds. The case report, published in Clinical Case Reports, described a 36-year-old woman who intentionally ingested approximately 40 bitter almonds in a suicide attempt. She developed severe cyanide poisoning, became unconscious and required emergency treatment, including intubation.²

This does not happen with regular almonds we buy. Sweet almonds available commercially have a very different safety profile. Viral posts claiming almonds have high levels of cyanide are incorrect.

Almond Allergy: When Even a Small Amount Is Dangerous

If you suffer from an allergy to almonds, then even a small amount can cause issues.

Food allergies are caused by the immune system reacting to proteins in food that it sees as a threat. This causes the immune system to release chemicals, causing symptoms such as itchiness, hives and digestive discomfort. Some allergies are not too severe; however, some can become life-threatening causing anaphylaxis. Anaphylaxis causes difficulty in breathing and can often lead to a sudden drop in blood pressure, requiring emergency care.

Almonds can be found in many different products such as chocolate, cereal and plant-based milks. If someone has been diagnosed with an almond allergy, they should avoid almonds completely. Be sure to check labels.

In India, the Food Safety and Standards Authority of India (FSSAI) lists peanuts and tree nuts, including almonds, among the recognized food allergens that must be declared on applicable packaged food labels under the category "Nut". Cross-contamination-related allergen declarations may also use precautionary statements such as "May Contain". People with almond allergies should check ingredient lists and allergen declarations carefully.3

Are Almonds Bad for Kidney Stones? Understanding Oxalates and Calcium

Almonds do contain oxalates. This natural compound is found in many plant sources. This can sometimes combine with calcium in urine to create kidney stones made of calcium oxalate. If someone is suffering frequently from kidney stones, it may be advised to be cautious of oxalate intake. However, almonds are not off limits.

The concern is particularly relevant to people who have a history of calcium oxalate kidney stones. The National Kidney Foundation identifies almonds as a food high in oxalates and recommends discussing possible dietary restrictions with a doctor or kidney dietitian.4

Whether someone suffers from kidney stones will depend on the type of kidney stones one has, their oxalate levels in urine and other factors. Interestingly enough, one should monitor calcium intake in such cases. Having enough calcium in our diet helps in binding the oxalate in the intestine.

According to the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), adequate calcium from food can bind oxalate in the digestive tract and reduce its absorption. Therefore, people with calcium oxalate stones should not independently reduce their dietary calcium intake.5

Unless told otherwise by a medical professional, it is rarely useful to cut out foods which are nutritious. If you suffer from recurrent kidney stones, speak to a urologist or renal dietitian.

Can Almonds Make IBS Symptoms Worse?

Almonds contain fiber and fermentable carbs. Galacto-oligosaccharides (GOS) are a type of FODMAP and are not ideal for those suffering from IBS. This can cause excessive bloating due to over-fermentation in the gut.

Monash University has guidelines for low FODMAP foods. Almonds are classed as a low FODMAP food. A typical serving is around 10 almonds (12g). This is a serving identified by Monash University as low in FODMAPs, not a universal dietary limit for everyone with IBS. 6 This does not mean that everyone with IBS should only have 10 almonds. However, if someone is suffering from digestion issues every time they have almonds, keeping track can help them work out how many almonds they can have.

Can People With Chronic Kidney Disease (CKD) Eat Almonds?

People suffering from Chronic Kidney Disease (CKD) often need to make dietary changes. This is because they may have reduced kidney function and need to be careful with certain minerals such as potassium and phosphorus.

In healthy kidneys these would be naturally regulated. However, with reduced kidney function, this can become an issue. While it would be misleading to assume that everyone with kidney disease should avoid nuts, everyone has an individual dietary requirement depending on how severe their CKD is, their blood tests and if they are having dialysis. The best option is to have a personal assessment from a renal dietitian.
