Apple has unveiled the Watch Series 11, its most advanced wearable yet, with two new features that could transform personal health monitoring: hypertension notifications and a sleep score system.

Hypertension Notifications: A Silent Killer Targeted

Hypertension, or high blood pressure, affects nearly 1.3 billion adults worldwide and is the leading modifiable risk factor for heart attack, stroke, and kidney disease. Often called the “silent killer,” it typically produces no symptoms and is frequently missed during routine clinical visits.

The new hypertension notification feature in Apple Watch Series 11 passively reviews cardiovascular data using the device’s optical heart sensor. By analyzing how blood vessels respond to heartbeats over 30-day periods, the watch can notify users if it detects consistent signs of high blood pressure.

Apple reports that the underlying algorithm was trained on data from over 100,000 participants and validated in a clinical study of more than 2,000 people. Though not a replacement for medical-grade cuffs, the feature is expected to alert more than 1 million people with undiagnosed hypertension in its first year of availability.

Experts say this could make a major difference in early detection. “Hypertension is the leading preventable cause of heart attacks and strokes, yet millions of people remain undiagnosed,” stated Dr. Harlan Krumholz, a cardiologist and scientist at Yale University and Yale New Haven Hospital, in a press statement from Apple. “Making accurate detection easy and part of daily life can help people get care earlier and prevent avoidable harm.”

The feature is currently under regulatory review, including by the U.S. FDA, with clearance expected soon. Once approved, it will be available in over 150 countries and regions, including the United States and European Union.

Brandon Ballinger, Co-Founder of Empirical Health, posted on LinkedIn on the underlying science of how this feature works:

Sleep Score: Turning Nights Into Data

Alongside hypertension alerts, Apple introduced sleep score, a feature designed to give users a clear picture of their nightly rest. The metric accounts for factors such as sleep duration, bedtime consistency, number of wake-ups, and time spent in different stages.

After each night, users receive a numerical score and a detailed breakdown in the Sleep app. The feature draws on more than 5 million nights of sleep data from the Apple Heart and Movement Study and reflects recommendations from the American Academy of Sleep Medicine, National Sleep Foundation, and World Sleep Society.

Users can view their sleep score directly in the Sleep app, add it as a complication on their watch face, or track long-term trends in the Health app on iPhone.

More Than Just Fitness Tracking

The new capabilities join a growing suite of Apple Watch health tools, including irregular rhythm notifications, high and low heart rate alerts, ECG, blood oxygen monitoring, retrospective ovulation estimates, and sleep apnea notifications. With these additions, the Series 11 strengthens its role as more than a fitness tracker, positioning itself as a personal health companion backed by scientific validation.

Apple emphasized that the Watch Series 11 is not a diagnostic device, but rather a tool to empower users with early insights so they can seek medical confirmation and, if necessary, treatment.

Availability

Hypertension notifications will be available on Apple Watch Series 9 and later, and sleep score will roll out with watchOS 26. Both features are expected to reach users this month.

Comparing Affordable Alternatives

Several companies have launched budget wearables that mimic some of these premium features at a fraction of the cost. Devices such as the YHE BP Doctor Fit (~$129) and the Fitvii GT5 Pro Max (~$165) claim blood pressure monitoring, ECG, SpO₂, and sleep tracking. Samsung’s new Galaxy Watch FE (~$199) and Apple’s own Watch SE (3rd Gen, ~$249) provide cheaper entry points to the ecosystem, though with reduced functionality.

Here is a feature comparison:

Do Cheaper Devices Hold Up?

Affordable wearables market themselves with claims of blood pressure measurement and sleep insights, but their validation remains limited. Optical sensors used by most budget devices often lack the clinical accuracy required for detecting hypertension. Apple’s advantage lies not just in hardware but in rigorous clinical validation, regulatory oversight, and integration with broader health studies.

For sleep tracking, budget models provide general insights, but Apple’s data-driven sleep score may offer more reliability, especially when considered alongside its FDA-cleared ECG and irregular rhythm notifications.

Final Word

Budget wearables can provide motivation for healthier habits at a lower cost, but when it comes to critical health features such as blood pressure and heart monitoring, validation and regulatory review matter. Apple Watch Series 11 sets a new bar with its hypertension notifications and evidence-based sleep scoring, features that could help uncover silent conditions earlier.

For those who simply want basic fitness tracking, cheaper alternatives are sufficient. But for users seeking clinically relevant health insights, the Series 11 makes a strong case — despite the higher price tag.
