Indian doctors who completed their internal medicine training in India may now have a new route to US board certification after the American Board of Internal Medicine (ABIM) introduced a special pathway for eligible international medical graduates. The initiative allows certain physicians to seek ABIM certification without completing the traditional three-year internal medicine residency programme in the United States or Canada.

The pathway applies to doctors who have finished at least three years of verified internal medicine training outside the US or Canada and later joined an Accreditation Council for Graduate Medical Education (ACGME)-accredited fellowship in an ABIM-recognised subspecialty. Candidates must also meet additional requirements, including holding a valid Educational Commission for Foreign Medical Graduates (ECFMG) certificate and satisfying ABIM licensure criteria.

ABIM Special Consideration Pathway Creates New Route to US Board Certification

The American Board of Internal Medicine (ABIM) has introduced Special Consideration Pathway E for international medical graduates who completed their internal medicine training outside the United States or Canada and later joined an ACGME-accredited fellowship programme.

Under ABIM's conventional route, physicians must complete a three-year accredited internal medicine residency in the US or Canada before qualifying for board examinations. The new provision gives eligible candidates an opportunity to seek certification based on their previous training and fellowship experience, provided they meet ABIM's requirements.

ABIM also states that doctors pursuing a subspecialty credential must first become board-certified in internal medicine before applying for a subspecialty examination.

Eligibility Requirements Under the New ABIM Pathway

According to ABIM's eligibility criteria, applicants must complete at least three years of verified internal medicine training outside the United States or Canada, hold a valid Educational Commission for Foreign Medical Graduates (ECFMG) certificate, and begin their fellowship training on or after July 1, 2016.

To qualify under the special consideration route, candidates must also:

  • Enter an ACGME-accredited fellowship in an ABIM specialty as an “exceptionally qualified candidate” under ACGME rules or as graduates of an ACGME-International residency programme.

  • Successfully finish all required subspecialty fellowship training, with clinical competence verified by the programme director.

  • Meet ABIM's licensure requirements when appearing for the Internal Medicine Certification Examination.

  • Submit supporting documents, including a letter confirming their overseas internal medicine training, a proposal letter from their fellowship institution, a current curriculum vitae, and a bibliography.

ABIM states that candidates approved under Pathway E can register for the Internal Medicine Certification Examination through the ABIM Physician Portal during designated registration periods.
