Dr. Jerri Nielsen was serving as the only physician at the Amundsen-Scott South Pole Station in Antarctica when she discovered a lump in her breast in March 1999. The station was isolated during the Antarctic winter, leaving her without access to a hospital or specialist on site.
Nielsen contacted physicians in the United States through satellite and email communication. With their guidance, she arranged a biopsy with help from station personnel and used a makeshift system to send images of the tissue to pathologists in the United States. The U.S. pathologists examined the transmitted biopsy images and confirmed that she had breast cancer.¹
She subsequently received cancer medicines delivered to the station by an aircraft airdrop and underwent chemotherapy while waiting for evacuation. In October 1999, she was flown out of Antarctica for further treatment.
Dr. Nielsen later shared her experience at the 25th Annual Meeting of the American Society of Breast Disease (ASBD). She also documented her experience at the South Pole in her 2001 book, Ice Bound, published by Talk Miramax Books.

Dr. Jerri Nielsen was an American physician who worked at the Amundsen-Scott South Pole Station during the 1998–1999 Antarctic season. She arrived at the station in November 1998 and served as its only physician.
The station operates at the geographic South Pole and becomes isolated during the Antarctic winter. According to CancerNetwork, aircraft generally cannot land there for about eight months of the year because of the extreme environmental conditions.
There is no way out. There is no way to get you anything. So, I didn’t expect any kind of rescue.

Nielsen was responsible for the medical needs of the station's winter-over crew. When she developed a potentially serious medical problem herself, she had to manage her care in the same isolated environment, but she was not medically unsupported. She remained in contact with U.S. physicians, including oncologist Kathy D. Miller, and received assistance from station colleagues who she trained to help with procedures and treatment.¹
In March 1999, Nielsen discovered a lump in her right breast. Over the following months, the mass increased in size and became hard and fixed. She also developed palpable lymph nodes under her arms and experienced breast and back pain.

For about three months, Nielsen kept her breast lesion known only to one trusted colleague at the station. During this period, she began preparing for the possibility that her condition could become fatal.
I started preparing for my own death.
She reorganized the station’s medical supplies into clearly labelled kits and copied relevant sections from medical textbooks, highlighting information that could help other crew members manage medical emergencies if she became unable to work. She also prepared brief notes covering medical situations that the crew might encounter in her absence.
CancerNetwork reported that the mass eventually measured approximately 5.5 cm. Nielsen contacted a physician friend in the United States, who connected her with oncologist Kathy D. Miller. The two began communicating remotely about her condition.
The doctors advised that she needed a biopsy to determine whether the lump contained cancerous tissue.
Then I kind of secretly started training people to take over for me, but I told them it was because I needed assistance. I taught them how to run the X-ray machine, how to run IVs, etc.
Because Nielsen could not leave the station at that time, she had to arrange the procedure locally.
She trained members of the station crew to assist with the biopsy.¹ The first biopsy did not produce a usable diagnostic result because the available staining material was old and did not work properly. New pathology stains were later delivered along with medical supplies. Nielsen repeated the biopsy after the supplies arrived.
A computer technician, Lisa Beal, at the station also helped create a system that combined a microscope, a camera and a computer. This allowed images of the biopsy samples to be transmitted to pathologists in the United States.

Nielsen's case involved remote communication between the South Pole station and medical professionals in the United States.
Her oncologist helped determine the treatment approach while Nielsen remained at the station. The AMA Journal of Ethics later described the case as telemedicine-assisted care in an extremely remote environment.¹
The technology did not replace the need for medical facilities or specialist care. Instead, it allowed physicians outside Antarctica to communicate with Nielsen and assist with decisions while she remained isolated.
Nielsen's care also depended on the people around her. A computer technician helped establish the system used to transmit biopsy images, while station personnel were trained to assist with medical procedures and treatment.¹ The station team also had to adapt equipment and procedures because conventional diagnostic and treatment facilities were unavailable.
After her diagnosis, Nielsen began cancer treatment while still at the South Pole.
The National Science Foundation arranged for chemotherapy drugs and other medical supplies to be delivered to the station by aircraft. CancerNetwork reported that the supplies included new pathology stains and chemotherapy medicines.
Her treatment initially included leuprolide and tamoxifen. She subsequently received paclitaxel, followed by a combination of methotrexate, fluorouracil and oral cyclophosphamide when the tumor began growing again.¹
Nielsen trained members of the station crew to assist with aspects of her treatment. A heavy-equipment mechanic and a machine operator helped administer chemotherapy, while another crew member assisted with intravenous access.
As her treatment continued, Nielsen developed significant symptoms, including dizziness, shortness of breath, back pain and problems with her veins. Her condition led her physicians to conclude that she needed to leave the South Pole.
The timing of her diagnosis created a major logistical problem.
During the Antarctic winter, extreme cold, darkness and weather conditions make flights to the South Pole difficult. Routine flights to the South Pole were not possible during much of the Antarctic winter, leaving the station effectively isolated for months.¹
For Nielsen, this meant that diagnosis and initial treatment had to take place at the station until an aircraft could safely reach her.
The AMA Journal of Ethics noted that even after she received a biopsy and chemotherapy, she still needed evacuation because the available technology and medical resources could not provide all the care she required.
Nielsen was evacuated from the South Pole on October 16, 1999.
CancerNetwork reported that the aircraft arrived earlier in the Antarctic season than previous rescue missions had typically been able to operate. The aircraft landed on an ice runway, took Nielsen aboard and left a replacement physician at the station before departing. Contemporary reporting described the rescue as an unusually early flight to the South Pole.³
After returning to the United States, Nielsen underwent surgery and additional treatment for breast cancer. She initially underwent a lumpectomy, developed a staphylococcal infection requiring rehospitalization, and later underwent a mastectomy as well as additional chemotherapy and radiation therapy.¹
Nielsen later documented her experience in the book Ice Bound: A Doctor's Incredible Battle for Survival at the South Pole, written with Maryanne Vollers and published in 2001.² published in 2001. The book described her diagnosis, treatment and eventual evacuation from Antarctica. Her story was subsequently adapted into a television movie.
She continued working as an emergency physician and spoke publicly about her experience. The University of Toledo, where Nielsen graduated from the Medical College of Ohio in 1977, reported that she continued to work and give talks after her treatment.
Her breast cancer later returned. The Guardian reported that the cancer had returned in August 2005. The University of Toledo later reported that the disease had spread to her liver and bones.
In October 2008, Nielsen addressed students at the University of Toledo College of Medicine, the institution where she had previously studied when it was known as the Medical College of Ohio. During the talk, she removed her wig to show the students her hair loss and encouraged them to continue contributing to the lives of others despite difficult circumstances.
There is no end of life until your last breath. We can all do something to help someone.
By March, Nielsen had told the Free Press that the cancer had spread to her brain. She continued writing, speaking publicly and participating in other activities despite the progression of the disease. She said that although her health and abilities had changed, she remained active and continued to find meaning in her work and daily life.

The things that make you strong, and make you feel as though you've accomplished something, are not the easy ones; it's the things you had to work and struggle through. Those are what give us our depth, that make us not gray and plain and nothing but give us depth and texture and longing.

Jerri Nielsen FitzGerald died from cancer on June 23, 2009, at her home in Southwick, Massachusetts. She was 57 years old.
Her husband, Thomas FitzGerald, confirmed her death. The Guardian reported that her breast cancer had returned in 2005, several years after her initial treatment.
She fought bravely, she was able to make the best of what life and circumstance gave her, and she had the most resilience I have ever seen in anyone. She fought hard, and she fought valiantly.

Dr. Jerri Nielsen’s story was not only about her battle with breast cancer, but also about receiving medical care under extraordinary circumstances. Isolated at the South Pole without access to conventional medical facilities, she relied on remote medical guidance, limited resources and assistance from station personnel until she could be evacuated.
Dr. Jerri Nielsen's case became an early example of how telemedicine could extend specialist support into an environment where conventional medical care was inaccessible. The technology allowed U.S. physicians to communicate with her and helped specialists assess biopsy images, but it could not replace definitive hospital care or evacuation.¹
Her experience showed how remote medical guidance, adapted equipment and assistance from trained colleagues could help provide care in an extreme environment, while also demonstrating the limits of technology when a patient ultimately requires hospital-based treatment.¹
